I plan to attend the Campbell Conference, July 16-18. ($25 or $50 check made out to CSSF)

Notes:

If you plan to enroll in the SF Teaching Institute, Writers Workshops, or other CSSF educational programs, use the full enrollment form.

Checks must be mailed in advance of arrival, made out to appropriate party.

You may combine checks for CSSF.

Housing cost checks are made out to CSSF.
